Each year, MID allocates a budgeted amount to community service activities for non-profit, non-political groups operating within MID’s electric service area. MID will donate one time per calendar year to each qualifying organization and/or program.
Donations usually range from $50 to $500, with an average amount of $250. In return for a donation, MID must receive some form of promotion for educational advertising or an opportunity to distribute MID educational materials.

Please complete all sections of this form and provide any necessary supporting documents. Organizations submitting a community service donation for the first time must also provide a description of the organization, a list of board members, and a complete W-9 form.
